What you suggest has totally been my pipe dream for a while (along with a "here's a guide to running the site" for people using DW installs in production), but we haven't had anybody who's been really interested in system documentation for a while, unfortunately!
What I've been pondering for a while is a sort of function-based "if you're trying to make changes to X workflow/user-facing page, you should look in the code in Y area" -- like, "if you're working on the navstrip, it's "control_strip" in cgi-bin/LJ/Web.pm", that kind of thing. But oi, the work involved. :(
